Schedule of Classes Instructor Change Form

Schedule builders must use this form to add, change, or remove instructors for sections after the SOC building deadline.

If a person has never before been entered as an instructor, TA, or D2L facilitator, an Instructor / Adviser / D2L Facilitator Role Maintenance form (PDF) and a signed CheckMarq Confidentiality Agreement (PDF) are required before he or she can be added to CheckMarq.

Please note: Changes to the Schedule of Classes not related to the instructor are not to be requested via this form.  All non-instructor changes to the Schedule of Classes must be made via the Schedule of Classes Change Request Form.
