If your 2016-2017 FAFSA is selected for Verification and you meet any of the below special tax filing statuses, you will be required to submit the following documentation to verify your reported FAFSA income data:

Tax Filing Status:

Then submit the following:

Filed a 2015 Federal Tax Filing Extension and have yet to file your 2015 Tax Return (See "Tax Extension" information)
Filed or Will File Amended Return
  • Signed copy of 2015 Federal IRS Tax Return Transcript; *and*
  • Signed copy of 2015 IRS Form 1040X, "Amended U.S. Individual Income Tax Return"
Filed Puerto Rican Tax Return Signed copy of the 2015 Puerto Rican Tax Return which can be ordered here: http://www.hacienda.gobierno.pr/ or by mail using Form 4506-T.
Filed Other Return: Guam, Virgin Islands, American Samoa, or Northern Mariana Islands Tax Return

Signed copy of the 2015 other Tax Return Transcript

* A signed copy of the applicable 2015 income tax return that was filed with the taxing authority is only acceptable if tax filers are unable to obtain a free copy of a transcript of their tax information

Filed Freely Associated States (Republic of the Marshall Islands, Republic of Palau, and Federated States of Micronesia) Tax Return

Signed copy of the 2015 other Tax Return Transcript

* A signed copy of the applicable 2015 income tax return that was filed with the taxing authority is only acceptable if tax filers are unable to obtain a free copy of a transcript of their tax information

Filed Foreign Tax Return

Signed copy of an official English translation of the 2015 Foreign Tax Return Transcript

* In cases where an individual from a foreign country, under that country's applicable government agency rules, is not required to file an income tax return, he or she can provide the institution with a signed statement certifying all of the individual's income and taxes paid for the tax year.

** A signed copy of the applicable 2015 income tax return that was filed with the taxing authority is only acceptable if tax filers are unable to obtain a free copy of a transcript of their tax information

Filed Married Filing Separately Signed copy of 2015 Federal IRS Tax Return Transcript for both tax filers
Married Filed Head of Household Signed copy of 2015 Federal IRS Tax Return Transcript
Victims of IRS Identity Theft
  • Signed and dated statement regarding the identity theft and attesting the filer notified the IRS about the theft; *and*
  • Signed copy of 2015 IRS alternate transcript, known as the TRDBV (Transcript Data Base View)

Filers who believe they are victims of identity theft may call Identity Protection Specialized Unit (IPSU) toll-free number at 800-908-4490 or go to the ID theft website on irs.gov. After the IPSU authenticates the tax filer's identify, the tax filer can request that the IRS mail to the tax filer an alternate paper tax return transcript.

Unable to obtain an IRS Tax Return Transcript using the IRS paper or online request after 10/18/2016

Note: for a tax filer who recently filed their tax return, six weeks of time should be allowed for the IRS to have processed the tax return before a tax transcript is requested
  • Signed copy 2015 IRS Income Tax Return and
  • Signed statement from the tax filer, on or attached to the tax return, which certifies that the tax return submitted to the institution includes the same information that was submitted to the IRS and
  • Signed and completed IRS Form 4506T-EZ or Form 4506-T that includes on line 5:

    Marquette University OSFA
    Zilber Hall Suite 121, PO Box 1881
    Milwaukee WI 53201-1881

    as the third party to whom the IRS is to mail the 2015 IRS Tax Return Transcript and
  • For tax filers who attempted to request an IRS Tax Return Transcript using:
    • IRS Form 4506T-EZ or Form 4506-T: Signed and dated copy of the IRS response mailed to the tax filer informing the tax filer that the IRS could not provide the requested transcript;

Note: Failure of a tax filer to successfully register for Get Transcript Online is not a sufficient reason. In such a case, a tax filer must request a tax transcript using Get Transcript by Mail or by using paper Form 4506T-EZ or Form 4506-T and provide documentation from the IRS that the tax transcript request was unsuccessful, if applicable.


If your marital status changed for the period January 1, 2016 through May 14, 2017 or if you are unmarried, living together, and unable to file a joint tax return, see the chart that follows.

  • Step-parent signed copy of 2015 Federal IRS Tax Return Transcript
Parent on the Dependent Student’s FAFSA filed a joint 2015 tax return, but is now separated, divorced, or widowed.
  • Signed copy of Parents' joint 2015 Federal IRS Tax Return Transcript
  • IRS Form W-2 for each source of employment income received for tax year 2015
Parents are unmarried, living together, and did not file a  joint income tax return
Independent student who filed a joint income tax return and who is married to someone other than the individual included on the joint income tax return
  • Signed copy of student’s joint 2015 Federal IRS Tax Return Transcript
  • IRS Form W-2 for each source of employment income received for tax year 2015
  • Signed copy of 2015 Federal IRS Tax Return Transcript for spouse
Independent student who filed a joint income tax return  who is separated, divorced or a widow of the individual included on the joint income tax return.
  • Signed copy of student's joint 2015 Federal IRS Tax Return Transcript
  • IRS Form W-2 for each source of employment income received for tax year 2015
