Health Risk Assessments
Health Risk Assessments will again be offered to all Marquette employees on a voluntary basis. For those who elect medical coverage, participation in the HRA will result in a cost-saving premium reduction for 2010. The HRA includes two parts: biometrics screening and online health questionnaire.
To schedule an on-campus Health Risk Assessment, please register online or call Aurora Total Health at (877) 765-3213 (Option #1).
Important Reminders
- All benefits will automatically rollover except for your 2010 flexible spending account election and any modifications you wish to make to other benefit plans.
- You must make any benefit updates through the MyJob site located at http://myjob.mu.edu. If you experience difficulty with MyJob, please read these Frequently Asked Questions.
- Please note, if you are enrolled in the health plan you must login to update your dependent(s) Social Security Numbers for 2010.

- Benefits take effect on your date of hire. No waiting period if you submit applicable benefit enrollment forms within 30 days of your employment.
- Full-time regular employees are offered health insurance, dental insurance, vision insurance, life and optional life insurance, short-term and long-term disability, and flexible spending accounts. TIAA-CREF 403(b) Marquette Match offered after two years of employment whereby the employee contributes 5% of gross salary and Marquette will contribute 8% of gross salary. (Subject to IRS maximums.) Voluntary participation in the supplemental retirement account is available the first of the month following the date of hire. Access to voluntary benefits and employee discount programs.
- Full-time temporary employees with a one year contract (academic or calendar) are offered health insurance, dental insurance, vision insurance, life and optional life insurance, short-term and long-term disability, and flexible spending accounts. TIAA-CREF 403(b) Marquette Match offered after two years of employment whereby the employee contributes 5% of gross salary and Marquette will contribute 8% of gross salary. (Subject to IRS maximums.) Voluntary participation in the supplemental retirement account is available the first of the month following the date of hire. Access to voluntary benefits and employee discount programs.
- Part-time regular employees are offered health insurance, dental insurance, vision insurance, life and optional life insurance, and flexible spending accounts. There are no premium contributions from Marquette University. Employees who participate in any of these benefits are responsible for the full premium. TIAA-CREF 403(b) Marquette Match offered after two years of employment whereby the employee contributes 5% of gross salary and Marquette will contribute 8% of gross salary. (Subject to IRS maximums.) Voluntary participation in the supplemental retirement account is available the first of the month following the date of hire. Access to voluntary benefits and employee discount programs.

- Marquette University retirees will be provided the option of continuing their desired coverage(s) by converting from a group policy to an individual policy. Rates are determined by MetLife and Northwestern Mutual.
- If you were hired prior to January 31, 1982, you may be eligible for a term basic life insurance policy at no cost to you. Contact a member of the benefits team for more information.
TIAA-CREF 403(b) Retirement Accounts
- You have the option of either leaving your funds in your 403(b) account upon your retirement until you wish to draw from your balance, rollover your funds into another account, or choose from one of several distribution options. Call TIAA-CREF at (800) 842-2776 for more information.

Social Security and Medicare
- We recommend that you attend one of the Social Security and Medicare sessions offered twice per semester. We will post more information on our main page as it becomes available.
- Contact a Social Security Administration office three months prior to your 65th birthday. The nearest office to Marquette University is the blue federal building located on 4th Street and Wisconsin Avenue. Appointments are recommended.

Internal Marquette University process to retire
- Give your department at least a two week notice. We recommend giving a two to three month notice to ensure all of your questions are answered and all of your benefit-related paperwork is processed prior to your actual effective date of retirement.
- Your supervisor or business manager will complete a "termination form"
- A member of the benefits team will receive your "termination form" and will contact you directly regarding retiree status eligibility and any applicable paperwork you may need.

Health Risk Assessments (HRA)
In 2008, Marquette University offered free Health Risk Assessments (HRA) to employees. The HRA consisted of a two-part process that included (1) a finger prick blood test that read glucose and cholesterol and (2) an online health questionnaire. Employees who successfully completed the two-part process were given a 5% discount on their 2009 health insurance premiums.
In 2009, we will offer a similar Health Risk Assessment process and health insurance incentive. Employees will need to participate again this year in order to be eligible for the discounted 2010 insurance premiums and both steps of the health risk assessment process are required to receive the discounted premium. All information is confidential, adhering to HIPAA and federal guidelines, and is for employees to use for health maintenance and preventive measures.
The 2009 Biometric Screenings have expanded to include triglycerides and LDL values. The additional, more accurate testing requires employees to fast for 12 hours prior to the blood test. Preparing for the HRA Process
You will need your Marquette University identification card (MUID) for all health risk assessment forms. Don't know how to find your MUID on your card? Click here.
You will also need to have access to your Marquette University email address. Contact IT Services at (414) 288-7799 if you do not have your Marquette email address set-up.
Step One | Biometric Screening
This includes checking height, weight, blood pressure, cholesterol and glucose. For the most accurate results, a 12 hour fast is required prior to your screening. Biometric screenings may be completed one of two ways no later than Nov. 20, 2009:
- Health care provider
- Biometric screenings performed at your primary care provider visit are valid from Jul. 1 through Nov. 20, 2009. Your office visit will be subject to any applicable co-pay.
- Download, print, and complete section one of the Health Risk Assessment Biometric Screening Primary Care Provider Form. Ask your primary care provider if they prefer receiving the form via fax, mail, or in person. You must request a copy of your biometric results, as you will need this to complete the online questionnaire offered during Annual Enrollment.
- Biometric screening results must be faxed to Aurora Health Care by Nov. 20, 2009. No exceptions will be made for forms received late.

Marquette University has partnered with the University of Michigan to offer a comprehensive Health Risk Assessment process for you to evaluate your health. This includes the biometric screening and online health
questionnaire. The online
questionnaire
creates a personalized report that helps to identify potential health concerns and lifestyle behaviors that may need to be changed.
The online health
questionnaire
will be available online Oct. 26 through Nov. 23, 2009.
The online questionnaire will ask for your Social Security Number. DO NOT enter your Social Security Number. Instead, enter your Marquette University Employee ID number. (Example: 000001234; must be nine-digits in length).
- Type in the Group Password = MARQUETTE
- Type in HRA Password
- This must be the same password as last year in order to compare your results.
- You may create a new password. Please remember your password for next year.
- Click on gender type and answer the smoking question.
- Complete questionnaire and print out report.
- Please note that you cannot change anything or go back into the questionnaire once you have clicked on "Submit".
It is important to retain a copy of this information for your records as proof of completion.
Seasonal Flu Shots
Seasonal flu and pneumonia shots will be offered on campus to all Marquette University employees on Oct. 26 and Oct. 27. Individuals that have the Marquette University health insurance will need to submit their name, health insurance member ID, and pay out-of-pocket for the service ($30 flu; $45 pneumonia). EPO plan members will be reimbursed 100% and PPO plan members will be reimbursed 80% by the end of the year.
No appointments are necessary.
