On-Line Co-op Checkout Packet

Co-op Checkout Instructions
before Leaving For Your First/Next Co-op Work Term:

Before beginning your first Co-op work term, all Co-op students MUST COMPLETE THE FOLLOWING: 

                        Co-op Agreement Form
                        Locator Card  
(for current address, e-mail information, etc.)

If you haven’t completed the above forms, please click on the name of the form and complete and submit them electronically or make an appointment with the Co-op Office immediately to do so.

After you complete your Co-op Agreement Form, you will be assigned to the following faculty Co-op advisors:

            Biomedical Engineering:                Ms. Laura Lindemann
            Civil & Environmental Engineering: Remain with your current advisor
            Computer Engineering:                 Professor Susan Riedel
            Electrical Engineering:                   Dr. Martin Seitz
            Mechanical Engineering:                Dr. Raymond Fournelle

Please remember that whenever you are in school during a fall or spring semester, you must see your faculty Co-op advisor during mid-semester advising week so you’re eligible to register for the next semester – even if the next semester is a scheduled Co-op work term for you.

EXCEPTION: In the summer, all Co-op students attending class must see Sue Michaelson to be advised for their next scheduled school term of classes.

Co-op Work Term Registrations (when you’re at work) are odd numbered:

Course Title 91 corresponds to work term 1
Course Title 93 corresponds to work term 2
Course Title 95 corresponds to work term 3       (All Engineering Majors)
Course Title 97 corresponds to work term 4      (BIEN, CEEN, COEN, EECE, MEEN)

Co-op Grading Credit Registrations (when you’re in school) are even numbered:

Course Title 92 corresponds to grading credit 1
Course Title 94 corresponds to grading credit 2
Course Title 96 corresponds to grading credit 3   (All Engineering Majors)
Course Title 98 corresponds to grading credit 4  (BIEN, CEEN, COEN, EECE, MEEN)

NOTE:    All students are responsible for registering for their Co-op work terms and grading credits via CheckMarq. A fee of $30.00 is charged for each work term. This fee will be billed to you by the Bursars Office. The Bursars Office will NOT allow students to register for work terms if the student owes money – contact the Co-op Office if this is a problem.

ALSO:  You do NOT pay for your 1-credit Co-op grading credit; it’s covered by your Co-op work term registration fee of $30.00 (this is important to remember when registering and paying for summer classes).
It is the Co-op student’s responsibility to always update his/her current street address, e-mail address and phone number information on CheckMarq.

Please also inform the Co-op Office – Each time you return to work or to school, please also inform the Co-op Office of this same updated information – susan.michaelson@marquette.edu – if your contact information (address, phone number and e-mail) has changed from the previous work or school term.

TO BE COMPLETED AT THE END OF YOUR WORK TERM

Employer Evaluation Form: To be completed by your supervisor immediately prior to completion of your work term. This form is electronic and can be found here.

Work Exit Report: To be completed by you prior to the completion of your work term. This form is electronic and can be found here.

Once these forms have been submitted you will be assigned one Co-op credit and a satisfactory grade for your work term.

Please take time to study your schedule. This is the schedule you will follow until you graduate.

INSURANCE
As a Co-op student you are covered by your parents’ insurance. However, if at any time you should need a form signed or a letter verifying your full-time status (i.e.: from your parents’ insurance company), please contact the Co-op Office with the following information:

  • Insurance Company Name / Address
  • To Whom Letter Should Be Addressed

FAFSA FINANCIAL AID FORMS
Although you’ll be co-oping on alternating semesters for three years, please remember to continue to complete your FAFSA forms each year!
